A licensee may use this form to request an official Letter of Certification for a District of Columbia Occupational or Professional License. The associated fee is $30 per Letter of Certification.

NOTE: Certifications are valid only if received from an official DC government email address (i.e., FirstName.LastName@dc.gov).

Please allow 5 - 7 days from the date of submission to receive your record certification.
