The purpose of this form is to initiate a financial review of funds paid to Occupational and Professional Licensing at the Department of Licensing and Consumer Protection to be considered for a refund. Refund requests are considered in cases of duplicate or overpayment, erroneous filing, withdrawn applications, and denied or unapproved license applications.

All refunds are returned to the credit card used to process the original transaction. With very few exceptions, a refund check may be issued.
